Position Summary
TASC Case Manager will provide comprehensive and specialized case management services for adults with identified behavioral health needs referred to TASC from the Will County Sheriff’s Office and/or Will County Courts. The role includes direct services and referrals based on identified needs of individuals enrolled into C‑VCSP services, including screening, assessment, referrals, and case management.

Essential Duties And Responsibilities

Screen, assess, evaluate, facilitate, and monitor individuals’ progress toward established goals and objectives.

Gather information from Circuit Court Clerk docket and failure‑to‑appear records.

Provide referrals for identified client needs.

Establish and maintain relationships with internal and external stakeholders, including the Will County Sheriff’s Office, pre‑trial services, public defender, probation, parole, and community support partners.

Build rapport with clients to develop a supportive and trusting relationship.

Manage a caseload of no more than 150 clients per year involved in the criminal legal system in Will County.

Engage clients within 2 business days of release from Will County jail and conduct check‑ins at least monthly, or as needed, via phone/video call, in‑person at the office, or in the community.

Support clients to ensure attendance at all court dates through reminders, transportation assistance, and removal of barriers to appearance.

Collaborate with defense counsel to provide necessary reports and letters of advocacy when requested.

Review program data regularly to meet performance goals and improve program quality for clients.

Demonstrate knowledge of DSM‑V and ASAM criteria.

Participate in staff meetings, monthly supervisions, and integrated staffing for clients receiving TASC services.

Complete and submit internal and external program reports.

Qualifications

High school diploma or GED equivalent.

Valid driver’s license.

Ability to obtain CADC certification within the first two years.

Proficiency in Microsoft Office Suite.

Ability to support a culture of diversity, equity, and inclusion.

Creative initiative and flexibility in leading group discussions.

Excellent verbal, written, and interpersonal communication skills.

Excellent time‑management skills with a proven ability to meet deadlines.

The agency currently offers hybrid work schedules that combine in‑office and remote work. Employees are required to report to the work site three days per week, with the option to work from home for up to two days remotely after 60 days of employment contingent upon job duties.
