Overview
PATH’s Intensive Case Management Services (ICMS) Program provides services to individuals experiencing homelessness with chronic medical conditions and co-occurring disorders who are high utilizers of the Department of Health Services (DHS) hospital and outpatient systems.
What is a Case Manager?
As part of the ICMS team, the Case Manager provides field-based, direct client services to program participants. Responsibilities of the Case Manager include:
Responsibilities
Providing case management services including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, and linkage to community providers for substance use, primary and mental health care
Collaborating with clients to develop individualized treatment plans that address short- and long-term goals
Providing ongoing case management support to assess plan progress and ensure goals are met or adjusted as necessary
Providing strengths-based case management and service coordination to assist clients in obtaining and maintaining stable housing
Maintain documentation standards as set forth by the program contract and PATH policies
What You Bring
Maintain and execute confidential information according to HIPAA standards
Demonstrate knowledge of or experience with evidence-based case management techniques including critical time intervention, trauma-informed care, harm reduction strategies, crisis intervention techniques and motivational interviewing
Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
Work independently as well as on a team
Exercise mature judgment, and be highly motivated, self-starting and proactive
Excellent communication, both written and verbal
Strong prioritization skills and ability to coordinate multiple demands in a high-pressure environment
Preferred Qualifications
Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field
Minimum Qualifications
All levels of education and experience are welcome
Mandatory Requirements
For this role the Case Manager must:
Be able and willing to work flexible hours which may include evenings or weekends
Have employment eligibility verification
Have or be able and willing to obtain CPR/First Aid training
Provide proof of full COVID-19 vaccination
Successfully complete the following as a condition of hire:
Tuberculosis Test
Background Screening
Drug Test
Driving
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
Have reliable transportation
A valid driver’s license
Proof of insurance and ownership for personal vehicles used during work duties
The ability to qualify for PATH's insurance coverage

California-Based Work Expectations
We are a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office does not establish approval for work to be conducted outside of California. PATH does not offer fully remote positions. Some roles may include a schedule that involves multiple designated worksites or approved off-site locations, but work will generally be on-site. Schedules are determined by responsibilities and may be adjusted.
