Description Program: EPIC
Pay Uncertified Case Managers start at $17.50. Uncertified Case Managers will obtain certification within the first 90 days and pay will be increased.
Certified Case Manager range starts at $19.61. Certified CM pay is calculated based on years of certified experience.
Shift Differential additional $2/hour when clocked in for 3+ hours between 3pm-11pm
Retention Sign-on Bonus $2000 for Full-Time
$1,000 for Part-Time
Schedule Sun-Thurs | 4pm - 12am

Job Summary & Deliverables The Residential Case Manager is a certified position that provides targeted case management and psychosocial rehabilitative services as indicated by the care plan to support clients in residential settings to meet individualized goals. Certified Case Managers utilize evidence-based practices, meet fiscal responsibilities, and ensure regulatory compliance and organizational standards.
Provide targeted case management services described by the Medicaid manual within residential programs
Support clients in following individualized care plans and identifying case management goals
Complete documentation on services provided within Valley’s standards
Distribute medications as necessary
Observe and collect client urine samples as necessary
Transport clients to appointments as necessary
Requirements Education High School diploma or equivalent

Licenses/Certifications Case Manager certification
CPR certification
Valley de-escalation certification
Driving positions require a minimum age of 21 and a current driver’s license
Preferred Qualifications One year social services or behavioral health experience
Bachelor’s degree in Social Services, Psychology, or related field
