Job Summary: The Case Manager/Swing Bed Coordinator is a blended clinical and administrative role serving the rural health clinics and hospital swing bed program. This position supports and grows programs such as Chronic Care Management (CCM), Transitional Care Management (TCM), Medicare Wellness Visits (MWV), and other initiatives as assigned. Responsibilities include coordinating care for patients, providers, and care teams to improve care transitions, enhance communication, and promote patient engagement. The Case Manager empowers patients to better understand the healthcare system, develop self-management skills, and actively participate in their care plans.
Key Responsibilities: Collaborate with the Primary Care Practitioner and care team to identify and engage patients for care coordination activities.
Develop individualized care plans with patients and the care team; assist with goal setting for self-management; review and update plans and goals as needed.
Maintain accurate documentation in the Electronic Health Record (EHR)and other required software systems.
Enroll eligible patients in Chronic Care Management (CCM)and Transitional Care Management (TCM)programs.
Ensure clinic patients complete Medicare Wellness Visits (MWV)and other preventive services.
Review the High Acuity list to identify patients appropriate for additional services.
Facilitate communication between patients, caregivers, and providers to improve care transitions, strengthen continuity, and enhance patient engagement.
Promote self-management skills and healthcare system literacy to empower patients, while identifying and addressing treatment gaps, duplication, or fragmentation.
Work to prevent adverse patient occurrences when possible; intervene promptly to minimize poor outcomes.
Complete audits as requested by the Clinic Manager, Clinic Director, or Director of Nursing.
Develop and maintain positive relationships with outside facilities, hospitals, and nursing homes to promote and grow the Swing Bed program.
Facilitate admissions, discharges, and overall management of Swing Bed patients.
Maintain accurate records of patient interactions, assessments, and progress.
Perform other duties as assigned.
Qualifications: Education: Graduate of LPN program or higher
Experience: 2 years of nursing experience preferred
Familiar with relevant healthcare regulations, guidelines, and payer requirements
Experience with Skilled Nursing Facility preferred
Certification/Licensure :Active RN or LPN License Required
