Job Responsibilities
Position Summary: Provide Case Management services and support to people who have a disability and/or medical need which require assistance with Activities of Daily Living (including bathing, dressing, toileting, eating, and mobility). Primary objectives include establishing positive working relationships with each person supported and their support system, exploring options and growth opportunities, and developing and monitoring Person-Centered Plans of Service (POS) that document needs, outcomes, and ongoing support. Ensure services meet state mandated standards and coordinate ongoing support services.
Essential Duties
Advocate for the people served, provide resource information, and educate them to become more self-determined.
Provide education to people served and their support system on coordinating and advocating for services.
Encourage self-empowerment and independence.
Educate on the principles of self-determination.
Collect information and documentation related to eligibility for Supports Planning services.
Meet with people served or their representative to obtain information and plan POS.
Develop and update Person-Centered Plans of Service with people served.
Plan, facilitate, and continually update POS tools documenting service needs.
Research, identify, refer, and coordinate resources to achieve POS outcomes; share resources with team.
Monitor POS and conduct follow-ups, including on-site visits and progress reviews.
Identify new services and assist with transitions between providers and programs.
Complete required paperwork and assist with waivers as needed.
Ensure HIPAA compliance and secure handling of records; meet performance utilization benchmarks.

Minimum Qualifications
Education: Bachelor’s or Master’s degree in a human service field (e.g., Human Services Administration, Psychology, Sociology, Social Work, Nursing, Counseling).
Experience: Experience with people with disabilities and with person-centered planning preferred.
Physical Demands
Generally, the role requires an office environment. Ability to operate a computer and move about an office, with potential lifting of 10–25 pounds. May require driving. Reasonable accommodations available for disabilities.
