General Statement of Duties:
The Home Base Case Manager Supervisor works under the supervision and direction of their supervisor to provide the highest quality of care to children and families. The role includes in-home and community-based family services such as crisis intervention, counseling, skill building, and advocacy. Maintains a flexible work schedule of 40 hours per week, manages emails, liaises with the Department of Child Services, Probation, and community resources. The supervisor is responsible for assigning referrals, reviewing and approving case notes, treatment plans, and reports for compliance, conducting chart audits, and managing caseloads to meet state audit requirements. Provides direct supervision to up to 5 staff and covers needs due to staff illness, injury, or absence.
Essential Functions:
II. Conduct interventions aligned with the agency service model and DCS Service Standards, including but not limited to:
Supervising up to 5 staff members
Assisting with weekly staff meetings
Reviewing all case notes, treatment plans, and monthly reports of assigned employees
Monitoring hours for timesheet reporting
Providing coverage if staff fail to provide services
Assigning and tracking incoming referrals within contract expectations
Scheduling and completing client intake within 48 hours of referral
Providing 24-hour crisis intake, intervention, and consultation, primarily in the family's home
Assessing and monitoring family/parental risk and progress
Delivering time-limited, family-centered, and child-focused services with measurable outcomes
Developing short and long-term family goals based on the case plan
Including intensive in-home skill building and after-care linkage
Providing monthly progress reports, supporting documentation, and court testimony as needed
Ensuring services are culturally competent and respectful of family values and complex interactions
Participating in Child and Family Team meetings and developing treatment plans accordingly
Conducting holistic, strength-based assessments
Maintaining face-to-face client hours at a 5/2 ratio, including non-billable duties
Offering clinical, concrete, and advocacy services tailored to family circumstances and culture
Responding to safety issues appropriately
Utilizing motivational, cognitive, and behavioral strategies and collaborative goal setting
Developing community resource linkages and ongoing service evaluations
Soliciting family feedback and managing intervention termination
Participating in additional program activities as directed
Completing all paperwork accurately and timely
Maintaining communication with supervisor and community partners
Upholding client confidentiality and agency policies
Engaging in community and interagency representation and relationship building
Other Essential Functions:
Maintaining prompt and regular attendance
Being on-call 24/7 for families
Arranging coverage during extended absences
Scheduling flexible hours, including evenings, weekends, and holidays
Traveling to client homes and community locations
Attending mandatory meetings and training
Setting and reviewing annual goals with supervisor
Participating in agency fundraisers
Job Qualifications:
Bachelor's degree in social work, psychology, sociology, or related field; MSW/MA preferred, with two years of social service experience
Valid driver’s license and insurance
Commitment to agency values and mission
Knowledge of child abuse, neglect, and development
Familiarity with community resources and team collaboration
Belief in client-centered change and adoption as a family-building option
Understanding of adoption-specific issues
Confidentiality and ability to work with diverse populations
Knowledge of crisis intervention, parenting, and behavioral strategies
Current First Aid and CPR certifications
Effective communication skills and proficiency with Microsoft Office
Residing in the designated service area
Knowledge of relevant laws and policies
Ability to assess safety and handle conflicts
Resilience to job-related stress and sound judgment
Working Conditions:

Travel required to client homes and community locations
Potential exposure to health risks and challenging situations
Physical Demands:
Proficiency with office equipment and computer use
Frequent standing, walking, sitting, and occasional lifting (up to 50 lbs.)
Other physical activities as needed for client support
