We are a large, non‑profit provider of behavioral health services with more than 115 sites and over 1,000 employees. We are looking for a SMI Case Manager to join our Metro Outpatient Clinic. The role involves providing direct services in the home, community, or office, including crisis stabilization, member assessment, case management, and coordination of care.
Responsibilities
Provide comprehensive in‑home and/or community‑based intervention to adult individuals with Serious Mental Illness.
Assess members in accordance with established policies, procedures, and clinical protocols.
Develop service plans based on assessment and diagnostic processes.
Coordinate member care, review interventions with care teams, and revise plans as needed.
Build and maintain relationships with members and support systems.
Offer education and guidance on treatment planning, advocacy, and medication management.
Monitor mental status and treatment response; update plans accordingly.
Notify supervisor of challenges such as decompensation, health concerns, or crisis situations.
Provide face‑to‑face and telephone crisis intervention.
Create safe/support plans for members.
Conduct groups when required.
Transition members to appropriate levels of care when treatment concludes.
Inform members and families about community resources and linkages to other services.
Assist families in securing appointments and support services.
Arrange transportation for community services, as necessary.
Maintain accurate, thorough documentation of member contact, treatment progress, and services received.
Coordinate with PCPs and health plans for all applicable populations.
Travel to conduct home visits and attend client appointments and community meetings.
Additional job duties may be assigned to meet program or department needs.
Qualifications
Bachelor’s degree with at least one year of full‑time behavioral health experience.
Equivalent combination of education and experience totaling five years is acceptable.
Fingerprint clearance issued by the Arizona Department of Public Safety (or eligibility within 8 weeks of hire).
Experience working with multicultural and low socioeconomic populations.
Valid Arizona driver’s license and access to a personal vehicle for business purposes.
Requirement to maintain personal auto insurance per SB&H guidelines.
Preferred: minimum of 2 years SMI PNO/IHH experience.
