Overview Case Manager - RN Weekend at Pascagoula Hospital. Full-Time position with weekend shifts (Friday, Saturday, Sunday) located at 2809 Denny Ave, Pascagoula, Mississippi, 39581, United States.
The Case Manager focuses on clinical quality and resource management resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care to a population of different ages within the Singing River Health System. All duties are performed in accordance with Singing River Health System procedures and policies, accreditation organization standards, and governing guidance for health care employees.
Case Manager must be able to work effectively under pressure, multitask, and be self-directed. Strong written and oral communication skills, analytical and critical thinking skills, conflict management and team-building skills, and strong organizational skills are required. The role involves participating in daily discharge planning meetings to address barriers to discharge and to ensure a seamless transition between levels of care, facilitating communication between inpatient care management and outpatient case management teams.

Education and Credentials Education: Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
Licenses: Current unrestricted Mississippi State licensure as a nurse.
Certifications: Must complete and maintain BCLS certification upon hire and/or transfer. Current TB certification as applicable to the position and scope of practice. De-escalation training must be completed by the end of position orientation (90 days) and maintained at the appropriate level.
Experience Two (2) years’ nursing experience in ambulatory or acute care that has relevance to the practice of Case Management.

Physical, Mental, and Special Demands Physical Demands: Work is moderately active; involves sitting with frequent movement, traveling within the SRHS service area, and handling objects and the human body as needed.
Environmental and Safety: Exposure to body fluids, sputum, and tissues with potential infectious disease; repetitive motions of wrists, hands, and fingers; use of standard office equipment.
Vision and Hearing: Must have normal to corrected visual and auditory acuity appropriate for the role.
Schedule: Must be able to be active for extended periods and work assigned schedules that may change for hospital needs, including off days, shifts, or weekends.
Mental Demands: Keen assessment and decision-making abilities, excellent communication, emotional stability, ability to work under pressure and meet deadlines, attention to detail, and ability to multi-task in complex situations.
Special Demands: Superior customer service skills, professional etiquette, computer proficiency (keyboard, MS Outlook and Word), and appropriate clinical judgment across all patient ages.
