Case Manager - RN Weekend
Location: Singing River Health System, Pascagoula, MS (2809 Denny Ave, 39581).
Position Overview: The Case Manager focuses on clinical quality and resource management to achieve clinical satisfaction and financial outcomes. Coordinate patient care for a diverse population within Singing River Health System. Perform duties in accordance with SRHS procedures, policies, accreditation, and governing guidance.
Key Responsibilities:
Work effectively under pressure, multitask, and be self‑directed.
Demonstrate strong written and oral communication, analytical and critical thinking, conflict management, team‑building, and organizational skills.
Perform numerous tasks simultaneously with minimal errors.
Participate in daily discharge planning meetings: address barriers that may inhibit discharge and target successful transition from one level of care to the next.
Create a seamless line of communication between inpatient care management team and outpatient case management team.
Qualifications:
Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
Current unrestricted Mississippi State licensure as a nurse.
Must complete and maintain BCLS certification upon hire and/or transfer.
Must maintain TB certification as applicable.
De‑escalation training completed by the end of position orientation (90 days).
Two (2) years nursing experience in ambulatory or acute care relevant to Case Management.
Must possess superior customer service skills, professional etiquette, and proficiency with computer and office technology (Outlook, Word).
Reports To: Director – Patient Resource Management.
Supervises: None.
Physical Demands: Moderately active; involves sitting, moving about facility, traveling to another facility within SRHS service area. Lifting, carrying, pushing, pulling objects; repetitive motions; exposure to body fluids.
Mental Demands: Keen mental faculties, decision making, superior communication skills, emotional stability, ability to work under pressure and meet deadlines. Attention to detail and multitasking.
Special Demands: Strong customer service skills, professional etiquette, computer proficiency, appropriate clinical judgment for patient population of all ages.
