Job Summary And Qualifications
The RN Case Manager is responsible for promoting patient‑centered care by coordinating the plan of care during the patient’s stay, managing the length of stay, ensuring appropriate resource management and developing a safe, appropriate discharge plan in collaboration with the multidisciplinary team.

Responsibilities

Provide case management services for both inpatient and observation patients as assigned.

Identify patients at risk for adverse outcomes during transitions between levels of care.

Perform comprehensive psychosocial, medical and discharge needs assessments of patients and families.

Reassess the patient’s clinical condition as indicated and develop strategies to mitigate risk of readmission.

Coordinate the plan of care and drive the discharge plan in collaboration with the multidisciplinary health‑care team.

Partner with Social Services to ensure appropriate post‑acute medical needs.

Assume responsibility for timely referral to Social Services when psychosocial risks are identified.

Involve patient and family in defining needs and expectations to develop realistic goals.

Evaluate progression of care using evidence‑based tools and approved criteria (InterQual) throughout the episode of care.

Escalate progression and transition of care issues through the established chain of command.

Make appropriate referrals to third‑party payers and disease and case management programs for recurring and chronic disease patients.

Facilitate patient throughput with a focus on care transition, quality and efficiency.

Document professional recommendations, discharge plan, care coordination interventions and case management activities.

Align patient needs with available resources to ensure a safe discharge/transition.

Practice and adhere to the “Code of Conduct” and “Mission and Value Statement.”

Qualifications

(RN) Registered Nurse.

Associate Degree, or Bachelor’s or Master’s Degree.

2+ years in case management or 3+ years of experience in clinical nursing.
