General Description Of The Position
The Case Manager at Sierra Meadows Behavioral Health is responsible for overseeing documentation of patient intake, progress, and discharge, as well as linking them with appropriate social, occupational, and educational resources. The Case Manager works well with the treatment team to plan and coordinate the client’s progress through the continuum of care.
Essential Functions
Patient Admission & Orientation – Admits new patients by completing the intake process and new patient orientation.
Assessment & Treatment Planning – Evaluates patients’ needs for medical, psycho-social, and psychiatric treatment.
Case Management & Patient Monitoring – Monitors cases by verifying attendance, evaluating progress, advocating for needed services, obtaining resources, and providing personal support.
Maintain patient records – Review case notes and document progress.
Communication & Interdisciplinary Collaboration – Participate in weekly interdisciplinary meetings, disseminate results and obstacles, identify treatment influences.
Discharge Planning & Post-Care Coordination – Prepare discharge plans, coordinate discharge and post-discharge requirements, orient and train family members, provide resources.
Prepare reports – Collect, analyze, and summarize treatment data and trends, compile statistics.
Staff Development & Training – Improve treatment results by evaluating and redesigning processes, implementing changes, rewriting policies and procedures; enhance staff competence by providing educational resources, balancing work requirements with learning opportunities, evaluating learning application.
Continual learning – Participate in educational opportunities, read professional publications, maintain networks, and participate in professional organizations.
Additional Functions
Perform other duties as may be assigned.

Travel occasionally during the workday and on overnight stays (compensation for mileage, food, lodging provided).

Requirements
Education: Bachelor’s Degree in Psychology, Social Work, or related field.
Experience: Two (1) years in a behavioral health care setting.
Skills: Strong leadership, communication, and organizational skills; ability to work independently and as part of a team.
Experience with culturally diverse individuals; commitment to diversity engagement.
Familiarity with equity and/or diversity initiatives.
Written and oral communication skills sufficient to perform essential functions.
