Job Description
The Idaho Youth Ranch is opening its new Residential Center for Healing & Resilience on the Hands of Promise Campus. As a Case Manager you will coordinate clinically aligned services to support youth and families receiving IYR services. The role supports the mission of nurturing hope, healing, and resilience.
Responsibilities
Provide case management for youth receiving inpatient residential care.
Assess, plan, link, coordinate, and monitor options and services that address a youth and family's needs.
Help clients maintain access to services and providers to achieve treatment goals.
Provide skill‑building services specific to goals identified in individualized treatment plans to ensure youth reach the least restrictive service level.
Collaborate with the clinical and medical team, client families, and referral sources to meet comprehensive client needs.
Qualifications
Social Work Licensure or a bachelor’s degree in behavioral science or related field with 1 year experience with children in a social service setting, or 5 years experience with children in a social service setting.
Ability to complete annual certification of CANS.
Demonstrated knowledge of Medicaid Youth Empowerment Services program.
Working understanding of HIPAA and Medicaid guidelines and ability to maintain youth confidentiality and privacy.
