The Case Manager plays a vital role in coordinating patient care by conducting comprehensive assessments, developing and implementing care plans, and evaluating outcomes to ensure optimal resource use and service delivery. Acting as a patient advocate, the CM addresses holistic needs through effective discharge planning and care coordination, while honoring individual preferences. This position integrates case management, utilization review, and discharge planning to enhance patient satisfaction, continuity of care, and cost-effectiveness. The Case Manager also upholds the hospital's mission, vision, and values in every aspect of care.

What You'll Do:

  • Conducts timely patient assessments, develops and documents individualized care plans, and ensures compliance with CMS guidelines and hospital policies.
  • Coordinates interdisciplinary team conferences, collaborates with staff to address care barriers, and communicates regularly with patients and caregivers.
  • Participates in performance improvement initiatives, utilization review activities, and communicates with insurance payers regarding clinical updates and appeals.
  • Manages comprehensive discharge planning, ensuring safe transitions of care and honoring patient choice across a range of post-acute services.
  • Identifies trends affecting care quality and cost-effectiveness, and contributes insights to leadership for continuous improvement.

Qualifications and requirements:

  • Current licensed Registered Nurse or Social Worker
  • Minimum of 2 years social work or case management experience preferred, preferably in an inpatient setting with discharge planning experience.
