Overview Rehabilitation Hospital of Montana, operated jointly with Lifepoint Health and Billings Clinic, invites a licensed Case Manager to join our inpatient rehabilitation team. The role focuses on coordinating interdisciplinary care plans, discharge planning, and patient progress communication to ensure timely, effective care transitions for individuals recovering from stroke, brain injury, neurological conditions, and orthopedic injury.
Responsibilities Serve as liaison among patients, caregivers, and the healthcare team to coordinate interdisciplinary care plans and discharge planning within the inpatient rehabilitation setting.
Attend departmental orientation, initial and annual competencies, and Complete documentation per workflow timeline and content requirements, including Individual Plan of Care.
Schedule family conferences and/or communicate with caregivers following each team conference and more often as needed to keep patient and caregiver informed of progress.
Coordinate weekly patient care team conferences and communicate effectively with nursing, therapy, and other departments.
Ensure clinical updates are provided to all insurance payers when due and document payer communications in Meditech.
Perform intake assessment on patient within 24 to 72 hours of admission and follow-up assessments per Case Management Plan.
Qualifications License required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech‑Language Pathologist.
