Overview Case Manager Rehab at St. David's HealthCare. Registered Nurse Case Manager Rehab opening with St. David's Medical Center. Apply to join a team delivering coordinated case management in the Rehabilitation Center.
Responsibilities Performs and provides professional case management services in the Rehabilitation Center to coordinate delivery of services and treatment planning.
Ensure and document effective cost management to maximize financial resources and utilize available resources.
Promote positive relationships between facility staff, patient and family, funding representatives, and other involved parties.
Ensure continuity of care post-discharge.
Assist in facility compliance with relevant standards of care and treatment.
Assist in development and implementation of new products and services, staff education and training, and good management practices.
What qualifications you will need Experience Required 1 – 3 years
The facility may substitute additional years of work experience for education at its discretion. A minimum of 1 year clinical experience in a physical rehabilitation setting/organization or 2 years experience in another related health care setting or any combination of the two.
Education Required Associate’s Degree - Major Nursing
Licenses and Certifications Licensed Registered Nurse, Therapist (Occupational, Speech, Physical Therapy) or Clinical Social Worker in the State of Texas
