Location : Mint Hill Medical Center - Mint Hill, NC; Mint Hill Medical Center is a 46 bed facility recently voted in the top 10 community hospitals in the nation. MHMC is a growing facility with a newly opened, large Emergency Department Expansion.
This role is ideal for an experienced acute care RN looking to work in a patient facing but non-bedside role within a high-performing, collaborative team.

What We're Looking For: Education: 2 Year / Associate Degree, required. Associates Degree in Nursing or BSN, required. 4 Year / Bachelors Degree, preferred.
Experience: Two or more years healthcare experience, preferred.
One year of Case Management or related experience, preferred
Licensure/Certification: Current RN licensure in state of employment, required.
Additional Skills Required: Weekend and holiday coverage, if applicable.
Additional Skills Preferred: Knowledge/Competence of Microsoft office products and keyboarding.
What You'll Do: Complex discharge planning
Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive/complex/safe discharge plans
Collaborate with MSW case managers for guidance with pscyhosocially complex cases
Facilitate coordination of care
Accept responsibility, authority, and accountability for management of care
Serve as a resource for discharge planning and care coordination for medically complex cases
Maintain current knowledge of available community resources/post acute care options
