Job Summary
The TCOOMMI Case Manager supports clients transitioning into the community after incarceration. Responsibilities include completing screenings, biopsychosocial/intake assessments, treatment plans, service coordination/case management, medication training and support, and living skills training. The role is primarily clinic-based with flexibility for community engagements when appropriate, collaborating with criminal justice partners (TDCJ Parole, Probation, Pretrial), and working with interdisciplinary teams to provide comprehensive care. The position reports to the Practice Manager and may support program reports, documentation reviews, compliance audits, and clinical staffing meetings.

Responsibilities

Conduct screenings, biopsychosocial/intake assessments, and develop treatment plans.

Provide service coordination, case management, medication training, and living skills training.

Coordinate with criminal justice partners and interdisciplinary teams for treatment planning.

Compile program reports, review documentation, conduct compliance audits, and lead clinical staff meetings.

Maintain trauma-sensitive, suicide-safe care and a supportive environment.

Qualifications
Education

Bachelor’s degree in Human/Behavioral/Social Services or a related field. Master’s preferred.

License

Current and valid driver’s license with a clean driving record.

Qualified Mental Health Professional (QMHP) designation.

Experience

Experience working with individuals diagnosed with mental illness, co-occurring substance use disorders, chronic health conditions, and/or IDD.

Proficiency in trauma-sensitive and informed practices, and suicide-safe care.

Work Hours & Compensation
Hours : Full-Time. Workdays : Tuesday – Friday. Workhours : 7:30 AM – 5:30 PM or 8:00 AM – 6:00 PM. Salary : $24.18 – $25.39 per hour based on education.
