The Case Manager/QIDP advocates for individuals with developmental disabilities by coordinating services and ensuring documentation and regulatory compliance. This role leads the development and implementation of service plans, maintains regular client and family contact, responds to crises, and monitors outcomes to support integration and well-being. Key duties include facilitating team communication, conducting site visits, leading meetings, mentoring staff, and ensuring alignment with agency goals and standards. While working for our agency you will obtain a QIDP Train the Trainer certification, Safety Care certification, First Aid and CPR certification.

Qualifications
Bachelor's degree (or higher) in social work, Psychology or a related field, as required by state regulations.

QIDP certification (40 hours of state required training)

Preferred Skills
Knowledge of theories, methodologies, technologies, and Federal and State laws and regulations pertinent to the field of developmental disabilities. Operate personal vehicle, as necessary, for transportation needs of persons served and/or movement between program/worksites. Utilize and operate Agency computer equipment for word processing/data management, telephone systems and other common office equipment.
