We are looking to add to our Prairieland family in Maryville, IL. Prairieland is seeking a Case Manager/QIDP to serve Madison and St. Clair counties. A Bachelor’s degree in a human services related field and one year of cumulative experience providing direct service to individuals with developmental disabilities are required. The one-year experience may be met through employment, volunteering, or family care. Excellent benefits, schedule 8:00 am to 4:30 pm, Monday through Friday, with the flexibility to work partially from home.
Overview Under the direct supervision of the Program Manager of Prairieland Service Coordination, Inc., the Case Manager assesses individual needs and ensures provision of necessary services through communication, service coordination linkage with provider agencies, service monitoring and individual advocacy.
Responsibilities Possibly work in counties outside of the caseload geographic area as needed (staff illness, staff resignation/termination).
Schedule and conduct intake/PUNS interviews in response to referrals, gathering initial information as required under the PAS Manual; conduct annual reviews as needed.
Complete the PAS process when eligible, following designated procedures and using supervisor assistance as appropriate to determine current availability of services.
Provide linkage to other community services for individuals found ineligible for PAS/ISC services, where possible.
Advocate on behalf of the individual and/or family to obtain services; ensure direct involvement and participation of the individual and family in the process.
Review medical, residential, vocational, educational, legal and financial needs and individual choices through Visiting Notes, as required.
Schedule and complete Discovery and Person-Centered Plan processes annually, or as required.
Document services delivered and maintain individual files as specified by program and agency guidelines.
Coordinate with provider agencies to resolve service coordination issues following ISC Conflict Resolution guidelines.
See all waiver individuals as required and complete program requirements in the waiver manual.
Coordinate specialized services for eligible Bogard individuals as needed and submit summary reports to external agencies as appropriate (if Bogard clients are on caseload).
Monitor services for Bogard class members monthly and advocate for unmet needs (if Bogard clients are on caseload).
Submit all required documents to the Program Manager for payroll preparation.
Participate in scheduled all-staff meetings and mandated training sessions as recommended by the Executive Director and/or Program Manager.
Alert the Program Assistant of possible issues, trends or service gaps.
Perform other related work as required or requested by the Executive Director and/or Program Manager.

Qualifications A bachelor’s degree in human services and QIDP eligibility as established by the Department of Human Services.
Ability to organize, plan and follow a schedule on daily, weekly, monthly and annual bases.
Working knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures.
Ability to work effectively with professionals, paraprofessionals, social service organizations and clients and their families.
Ability to work well in teams and to work at a home office site with self-discipline and self-motivation, with the ability to work without direct supervision.
Flexibility to work hours outside of the normal workweek.
Ability to cooperate with others toward a mutual goal within Prairieland Service Coordination, Inc. objectives.
Willingness to follow directions, accept constructive criticism and take responsibility for actions.
Professional conduct in accordance with the Employee Code of Ethics Guideline through diplomatic oral and written communication.
Must have reliable transportation to and from work and to places where individuals live/work that may be assigned; possess a valid Illinois driver’s license, personal auto insurance, and maintain a good motor vehicle record.
Physical Expectations Must be able to read, write and communicate effectively.
Must be able to ambulate, sit and bend; able to carry files.
Must be able to lift up to 5 lbs. or pull a carrier.
