Join Our Team as a Case Manager ! Are you passionate about supporting the Children and Youth population and making a real difference in their lives? EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated part-time Case Manager to join the Career Services Program.
Wage: $25/hour
Schedule: 19 hours per week. Mon, Tues, and Thurs. Available for morning and afternoon hours.
Location: Central Islip, NY
Reports to: Program Supervisor
Core Purpose of the Job The Part-time Case Manager within our Safe Harbour program provides trauma-informed, case management, and direct services to minor victims of human trafficking and youth at risk of trafficking, ensuring comprehensive support and response.
Key Responsibilities Provide trauma-informed, victim-centered case management and outreach to youth in Suffolk County, NY.
Conduct assessments and develop individualized service plans.
Transport and escort clients to court, medical appointments, and other necessary appointments.
Assist with applying for public benefits, employment, medical care, and coordinate services.
Make referrals for medical exams, mental health services, residential assistance.
Facilitate group services using an evidence-based curriculum to support trafficked or at-risk youth.
Document services in the CAC database for accurate case records.
Maintain a database of professional referral resources.
Provide backup and support for office coverage.
Prepare monthly, quarterly, and annual reports as required.
Required Knowledge, Skills, and Abilities Bachelor’s degree in Social Work, Forensic Psychology, or behavioral health; Master’s preferred.
Knowledge of human trafficking, child abuse, trauma, and exploitation.
Proficiency in computer applications, spreadsheets, Microsoft Office, and database software.
Excellent communication, organizational, and interpersonal skills.
Access to a vehicle and a valid NYS driver’s license.
