Division Overview: The Corrections Transition Programs (CTP) is a statewide network of clinical reentry management services that helps individuals prepare to return to families and communities after incarceration. CTP provides reentry support inside prisons and in the community, including behavioral health referrals, public benefits enrollment, employment and housing resources, and obtaining state IDs and vital records.
Starting salary: $44,646 - $47,650; contingent upon experience, education, etc.
Note: This role involves providing addiction intervention, AODA counseling, and recovery support services to pregnant and post-partum mothers and their families.
Responsibilities Assist individuals in achieving service plan goals through direct services and coordination of behavioral health, public agencies, and community resources.
Assess, evaluate, facilitate, and monitor progress relative to the client center service plan and conditions of parole.
Complete intake packets for program enrollment and participation.
Provide referrals and crisis intervention for identified needs.
Interview individuals, review records, and confer with other professionals to review progress and determine suitability for program participation.
Collaborate with internal and external partners, attend staff meetings and monthly supervision, and participate in integrated staffing with individuals receiving TASC services.
Identify and refer individuals with behavioral health needs.
Complete and submit internal and external program reports.
Maintain all required documentation including screenings, enrollments, service plans, progress notes, discharge summaries, and other related activities with accurate reporting of services.
Monitor individual progress toward service goals.
Assist clients with enrollment in public benefits systems and obtaining vital records.
Monitor TASC and external database systems to ensure program compliance.
Provide information on child development and parenting.
Lead small groups and follow curricula involving trauma, healthy relationships, co-parenting, and other topics as needed.
Qualifications / Ideal Candidate High school diploma or GED
Valid driver's license
CADC certification within two years of employment with TASC may be required based on contractual obligations
Ability to pass an IDOC background check
Proficiency in the Microsoft Office suite
Experience in group facilitation
Knowledge of ASAM placement criteria, DSM-V related to substance-related disorders, and ethics for substance abuse professionals (or ability to acquire)
Comfort working inside a correctional setting and with individuals convicted of serious offenses
Flexible thinking and approaches to leading group discussions
Strong verbal, written, and interpersonal communication skills
Good time management and ability to meet deadlines
