Overview PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Outreach Services team as the Case Manager Outreach - Rapid Response at the San Diego office. Looking for candidates who have specific outreach case management experience or related lived experience.
PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. Since its foundation in 1984, PATH operates services throughout California, connecting clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.

Shifts: Sunday-Thursday 10am-6:30pm; Tuesday-Saturday 10am-6:30pm

What You Will Do (Responsibilities) Conduct intake and individualized needs assessment for all clients and collaborate to develop an Individualized Service Plan (ISP) addressing housing goals, income goals, and other client-identified needs (medical, mental health, substance use, financial resources, vocational, and social support).
Provide referrals, linkages, information, and support to resources to help clients achieve ISP goals.
Complete program-specific assessments (e.g. VI-SPDAT).
Evaluate strengths and challenges to address short-term and long-term goals; conduct a 90-day review with the client after initial intake.

Preferred Qualifications Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations; outreach experience or related lived experience strongly preferred.

Minimum Qualifications All levels of education and experience are welcome.

Driving Driving is an essential function of this position. Candidates must:
Have reliable transportation
Have a valid California driver’s license
Provide proof of insurance and ownership for personal vehicles used during work duties
