Options Counselors assist older adults and individuals with disabilities in making informed choices about long-term care and support options. It involves an interactive, person-centered approach where individuals, along with their families or caregivers, discuss their needs, preferences, and values to determine the most appropriate long-term care choices. The counselor helps individuals understand their options, explore different settings and services, and make informed decisions.
Responsibilities Provide information and education to older adults and individuals with disabilities about long-term care options.
Conduct person-centered assessments with individuals and (where appropriate) families or caregivers to identify needs and preferences.
Assist clients in evaluating options, settings, and services and support informed decision-making.
Document assessments and recommendations accurately in client records.
Qualifications Bachelor\'s degree and two years\' relevant experience in human services or health care.
Excellent interpersonal, oral and written communication skills.
Commitment to working with the elderly and disabled adults; sensitivity to aging and disability issues.
Crisis intervention and problem-solving skills.
Ability to apply professional social work practices, principles, and techniques.
Leadership ability; independent thinker; self-starter.
Proficiency in Microsoft Office and experience using databases.
