Position Summary
This Case Management position is with Aetna’s Long‑Term Services and Supports (LTSS) team and is a field‑based position out of Harrisonburg, Virginia. The role requires a Virginia RN or LPN license, and requires traveling up to 50‑75% of the time to meet with members face to face.

Responsibilities

Assess medical needs and eligibility.

Develop care plans to address short‑ and long‑term health goals.

Use clinical tools and data to guide decisions.

Coordinate services across programs and providers.

Address complex health and social issues.

Review claims and functional capacity.

Refer members to appropriate clinical resources.

Collaborate with supervisors and multidisciplinary teams.

Ensure compliance with policies and regulations.

Engage members through effective communication.

Work Location & Travel Requirements
This is a Work‑at‑Home (WAH) position based out of the candidate’s residence in Harrisonburg, VA, with up to 75% field travel. The role requires a secure, HIPAA‑compliant home workspace with reliable phone and hard‑wired internet service. Work hours are Monday through Friday, 8 AM to 5 PM EST. Colleagues must be accessible via phone, videoconference, and email during scheduled hours and ensure that personal responsibilities, including dependent care, do not interfere with work duties.

Required Qualifications

Candidates must reside in or near Harrisonburg, VA.

Must possess reliable transportation and be willing and able to travel up to 50‑75% of the time. Mileage is reimbursed per company expense reimbursement policy.

LPN/RN with current unrestricted Virginia state licensure required.

3 years clinical experience (preferably in medical‑surgical nursing, behavioral health, long‑term care, etc.).

1+ years managed‑care experience.

Position requires proficiency with computer skills, navigating multiple systems and keyboarding.

Preferred Qualifications

Case management in an integrated model preferred (Waiver and IDDP–Combination of different services).

Bilingual in English/Spanish.

Education
Associate’s degree in Nursing required; BSN preferred.

Technology Skills

Proficient in multisystem navigation: comfortable using multiple monitors and systems simultaneously.

Effective multitasker: skilled at talking and typing concurrently while managing various responsibilities.

Basic technical troubleshooting: able to set up and connect essential hardware and resolve common technical issues independently.

Digital collaboration & documentation: experienced in electronic documentation and virtual collaboration tools.

Highly organized & results‑oriented: maintains a structured approach to work, ensuring tasks are completed on time.

Metric‑driven performance: demonstrates accountability by consistently meeting or exceeding established goals and expectations.

Clean & efficient workspace management: keeps workspaces tidy and organized to support productivity and comply with professional standards.

Pay Range
The typical pay range for this role is $60,522.00 – $129,615.00. This pay range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short‑term incentive program in addition to the base pay range listed above.
