Summary
Provide rehabilitative case management (RCM) for individuals served by Gulf Coast Center (GCC). Coordinate and implement Texas Resiliency and Recovery (TRR) RCM as part of an interdisciplinary team. Deliver clinical services, crisis resolution, skills training, and collaborative case management to support recovery and wellness.

Job Duties

Identify and manage a caseload in collaboration with the interdisciplinary team.

Provide initial screening and intake interviews to assess priority populations.

Coordinate patient workflow, complete crisis screening, and provide referrals to internal and external resources.

Develop person‑centered recovery plans and present cases to prescribers.

Deliver RCM self‑management strategies, crisis services, and monthly documented face‑to‑face hours per GCC guidelines.

Maintain compliance with documentation policies, complete Adult Needs Strengths Assessments (ANSA), and participate in quality assurance training.

Participate in Medicaid administrative claiming and track vital health indicators as part of CCBHC & 1115 Waiver outcome measures.

Collaborate with staff, consumers, and community partners to provide effective services and uphold GCC compliance policies.

Supervision
Receives direct supervision from Program Manager‑MCSC.

Required Education

Bachelor’s degree in behavioral sciences or related field from an accredited college or university and/or specialized QMHP certification.

Preferred Experience

Two years full‑time experience in human services.

General knowledge of mental disorders and strong interpersonal skills.

Demonstrated ability to operate video/virtual communication and work within a team approach.

Working Conditions

Office, field and/or remote settings.

Varied work hours and travel to Brazoria and Galveston counties for training.

Current Texas driver’s license, reliable transportation, and liability insurance required.

Physical/Mental Requirements and Environmental Conditions

Transport items up to 55 lbs, operate office equipment, and work long or irregular hours.

Maintain regular attendance, communicate in English, and solve practical problems.

Pass and maintain all Center‑mandated trainings.

Work alone or with clients and family members in environments that may include hostile or high‑crime conditions and exposure to tobacco products.
