Overview Employees of All of God’s Children will participate in the Core Practice Model, to better integrate engagement, teaming, assessing, treatment planning, and intervention services that support children, clients, families and communities, for the purpose of providing responsive, efficient high quality services that promote safety, permanency, well-being and self-sufficiency and are grounded in community partnership, teamwork, cultural competence, respect, accountability, continuous quality improvement and best practice.
Responsibilities Serve as the hub of the process as CFTM Facilitator and collaboratively orchestrate the development of the Individualized Child and Family Plans.
Assist in the completion of the following for each client:
Assist with client intake: email required documentations before client arrives.
Ensure client comes with medication.
Ensure client has a current JV223.
Assist with the client’s transfer plan.
Keep all required LIC State forms completed and updated.
Ensure all client’s dental, physical, and optometrist appointments are completed within 30 days of arrival.
Transport all clients to their dental, physical, and optometrist appointments. If there is a conflict with appointment times, assistance will be provided.
Schedule client visits and supervise all client visits and phone calls. If there is a conflict with appointment times, assistance will be provided.
Complete Monitored visit forms.
Ensure all clients review and sign AGC Wellness Packet.
Schedule all psychiatric medications to be refilled every 30 days.
Ensure clients are offered opportunities to participate in community recreation activities.
Handle all education/school matters: discipline problems, IEP, SST, BHP, RSP.
Assist with clients ICMP.
Have the clients complete quarterly surveys.
Update Quick References.
Handle all transportation to urgent appointments.
Assist the Social Worker with TBS service for clients.
Ensure the calendar is current for all needed obligations to efficiently run the Mental Health clinic.
Put non-clinical client information on the Needs and Service Plan.
Take the clients’ height and weight every 30 days.
Spend 8 hours per week on the floor; these hours will be broken up throughout the work week.
Other clerical duties as assigned.
Qualifications Bachelor of Arts Degree in Arts or Sciences.
Physical Demands Requires frequent sitting, standing, bending and reaching.
Requires manual dexterity to operate computer keyboard, calculator, copier machine and other office equipment.
Requires working under stressful conditions and working irregular hours.
Requires the ability to physically restrain clients.
Requires full range of body motion including handling or lifting at least 25 pounds.
Requires hearing within normal range for telephone use.
Requires eyesight correctable to normal range to operate a vehicle.
