Overview Case Manager Mental Health Professional BH -Bilingual (53844) at Association for Individual Development. The role involves delivering case management and mental health services to clients, with bilingual capabilities required.
Responsibilities The Case Manager MHP is responsible for developing and implementing individual treatment plans utilizing the IM+CANS to maximize the clients’ independent community living skills.
Complete documentation of mental health services provided in accordance with state and agency standards as applicable.
Collaborate with a team of behavioral health service providers within the agency to ensure a positive client-centered approach.
Perform remote work components as needed, subject to performance and operational needs; remote work is discretionary and may be rescinded at management’s discretion.
Schedule hours flexibly to meet client and program needs; provide independent program coverage when scheduled.
Ensure compliance with agency, state and federal regulations; review and follow updated policies and procedures.
Facilitate communications and coordination of services with other AID staff and community professionals via phone and email.
Maintain professionalism and appropriate boundaries with clients, coworkers and outside agencies.
Meet minimum service hour productivity standards (MRO) monthly and complete all required case management documentation (IM+CANS, consents, residential/agency paperwork) timely.
Complete and sign MRO documentation within 48 hours using Cx360; meet with assigned clients regularly and submit daily activity logs.
Develop, review and revise IM+CANS and complete corrections within the allotted timeframe.
Assist clients with activities of daily living, including apartment maintenance, and provide client training aligned with treatment goals.
Provide guidance to MHP and RSA staff regarding IM+CANS goals/objectives; obtain input from clients, families, guardians and other staff to improve residential services.
Monitor and report safety of clients’ living arrangements; obtain and maintain required trainings and certifications; assist clients with paying bills, budgeting and banking per program procedures.
Oversee or manage client funds when necessary and obtain/maintain client benefits (Social Security, Medicaid, Link Card, etc.).
Assure client records are properly maintained per agency procedures; complete authorizations, reauthorizations and spend-down paperwork promptly; update Cx360 as needed, at least annually.
Provide effective services aligned with client rights and the Mental Health Recovery Model; attend monthly clinical supervisions and team meetings; participate in agency activities as appropriate.
Recognize emergencies and take appropriate action; coordinate with management to complete incident reports and related paperwork; promote connections with clients’ families and supports.
Provide transportation to clients when necessary; maintain professional relationships with coworkers, agencies and stakeholders; maintain a team approach to services and client rights.
Participate in agency committees as assigned; coordinate community services to meet client needs (financial, legal, advocacy, medical, etc.); promote community/family involvement in facility programs.
Participate in fundraising and annual events; plan and implement client activities; request and track PTO through PayCom; notify management of any scheduling issues.
Perform other related duties per AID policies and philosophies; promote client wellness through coaching, counseling, consultation, motivational support, and information/referral services.
Communicate with service delivery partners (Physicians, Nurse Practitioners, Counselors, Pharmacists, Employers, Community Supports, other Health Professionals) to support wellness objectives; utilize health education resources as appropriate.
Engage diverse resources to support client wellness and respect diversity among clients, employees, families and stakeholders.

What you will bring to the table Education Bachelor’s degree in human services or related field required
Experience Some experience working in social services required
Bilingual experience required
Physical Navigation of stairs
No lifting restrictions
Ability to provide services in clients’ homes
Equipment Computer with Microsoft Windows applications
Copy machine; telephone with voicemail
Basic household appliances
Additional Requirements Use of personal automobile, valid driver’s license, and minimum liability insurance as defined by AID’s Personnel Policy
Drive self and clients in agency or personal vehicle
Must acquire and maintain certifications in First Aid, CPR, Non-violent Crisis Intervention training, CEUs and other relevant trainings
Evening hours may be required
Must be able to drive a passenger vehicle; must maintain IM+CANS certification
