Position Overview
The Case Manager focuses on clinical quality and resource management resulting in defined clinical satisfaction and financial outcomes. They coordinate patient care for a diverse patient population within the Singing River Health System. All duties are performed in accordance with system procedures, policies, accreditation standards, and healthcare regulations.
Case Managers must work effectively under pressure, multitask, and be self-directed. Strong communication, analytical, critical thinking, conflict management, team-building, and organizational skills are essential. They participate in daily discharge planning meetings to address barriers and facilitate patient transitions, ensuring seamless communication between inpatient and outpatient teams.
Education
Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
License
Current unrestricted Mississippi State nursing license.
Certification
Must complete and maintain BCLS certification upon hire, and TB certification as applicable. De-escalation training must be completed within 90 days of start date.
Experience
Minimum of two (2) years’ nursing experience in ambulatory or acute care relevant to Case Management.

Physical Demands
Moderately active work involving sitting, moving about the office and facility, and traveling within the SRHS service area. Must be able to exert force to lift, carry, push, or pull objects, including patient handling. Exposure to body fluids and tissues is possible, with associated infection risks. Repetitive motions and fine motor skills are required for operating office equipment. Visual and auditory acuity are necessary for effective communication and data review. Extended activity periods and schedule flexibility are expected.
Mental Demands
Strong assessment and decision-making skills, excellent communication abilities, emotional stability, and the capacity to work under pressure and meet deadlines are required. Attention to detail and multitasking in complex situations are essential for success. Maintaining respectful and collaborative relationships across teams is expected.
