Who we are looking for
We are looking for a compassionate and skilled Case Manager to join the Local Link pilot program, supporting victim-survivors of family, domestic and sexual violence in the Flinders and Upper North region. You’ll work closely with GPs, community services, and support networks to provide trauma-informed, culturally safe case management and strengthen local referral pathways.

To be successful in this role you will:

Tertiary qualification (AQF level 7) in social work, psychology, counselling, allied health, nursing or related discipline.

Demonstrated experience in providing case management, client support, or service navigation within health, community, or social service settings.

Strong understanding of family, domestic and sexual violence (FDSV) and child sexual abuse (CSA), including impacts on victim-survivors and their families.

Bonus: Knowledge of local service systems, referral pathways, and community resources in the FUNLHN region.

Bonus: Previous experience as a Nurse, Allied Health Practitioner, or other health professional within the primary care sector.

What you will be doing
You’ll act as a vital link between primary care providers, RASA, and specialist services—strengthening referral pathways and promoting integrated, holistic care for those affected by FDSV. You will work in close partnership with local GP clinics, Country SA PHN, and community services, you’ll provide short-term, trauma-informed case management and service navigation. Your role includes conducting risk and safety assessments, developing safety plans, and coordinating referrals to ensure timely, culturally safe, and client-centered support.
