Job Description
The HCB Waiver Case Manager is responsible for providing case management services to individuals enrolled in the Home and Community-Based (HCB) Waiver program. This role involves developing and monitoring clients' budgets and service plans, ensuring clients receive appropriate services, and maintaining compliance with program regulations.
Essential Job Functions
Assessment and Planning
Conduct initial assessments to determine clients' eligibility for the HCB Waiver program.
Develop and maintain individualized care plans based on clients' needs and preferences.
Coordinate reassessments to maintain clients' eligibility and update care plans as needed.
Service Coordination
Coordinate and monitor the delivery of services to ensure they align with clients' care plans.
Assist clients in accessing community resources and services.
Advocate for clients' needs and ensure their health, safety, and welfare.
Documentation And Compliance
Maintain accurate and up-to-date case notes and documentation in the Medicaid Waiver Management Application (MWMA).
Ensure compliance with federal and state regulations, including completing required paperwork and reports.
Review and approve timesheets, service documentation, and prior authorization letters.
Client Support
Conduct monthly home visits to monitor service delivery and progress toward clients' goals.
Provide support and guidance to clients and their families.
Collaboration
Work closely with traditional provider agencies, local departments, and community organizations.
Attend staff meetings and job development trainings.
Assist in coverage of caseloads when other staff are unavailable.
Education
Bachelor's degree in health and human services field from an accredited college or university; or at least 1 year of experience in a health of human services field; or the educational or experiential equivalent in the field of aging or disabilities.
Skills
Strong communication and organizational skills.
Proficiency in computer applications and the ability to multi-task.
Ability to build relationships and advocate for clients' needs.
