Case Manager III - Shared (On-Call) - evenings

The case manager will serve as direct care staff and case manager in several residential programs that serve clients who are trauma survivors, those struggling with substance abuse or mental illness, and victims of intimate partner violence. Programs include substance abuse treatment centers for women with children, emergency shelters for survivors of domestic violence and human trafficking, and a short‑term housing program for individuals with mental health and substance abuse disorders. This position provides supervision and support to residents at the assigned location as part of the 24‑hour direct care staff team in six residential programs.
Compensation: $25.00 per hour
Work Schedule: Monday through Sunday, evenings
Minimum Qualifications
Bachelor’s Degree in Social Work or Human Services field, and at least two years of direct care experience in Human Services (preferred); equivalent employment experience may be considered.
Valid Driver’s License with an acceptable driving record.
Essential Duties And Responsibilities
Provides supervision and support for program participants as part of residential program staffing for 24‑hour, 7‑day per week schedule.
Responds to all crisis calls, assesses the situation, gathers information and provides appropriate and accurate referrals, maintaining strict confidentiality.
Assists with meeting basic needs in several residential programs; including development and revision of ongoing safety plans and connecting clients to medical, therapeutic, or other services identified through the intake and assessment process.
Provides client transportation in personal or agency vehicles.
Follows program policies and procedures, including agency protocols related to medication administration.
Documents client activity, progress and needs per established record‑keeping procedures.
