The Case Manager has the responsibility for providing case management services to individuals of all ages with a primary diagnosis of intellectual and/or developmental disability. This includes assessing service needs; developing plans for access to services; liaison with client, families, programs, and service providers; ongoing monitoring of client service needs; advocacy; and consultation and education to clients, families and the community.
The Case Manager will provide services in Fluvanna and Louisa counties. They will report to the Rural Services ID/DD Program Manager for clinical supervision. The designated county Director III will provide administrative supervision for this position. The Case Manager is expected to function with initiative and independent judgment, based on application of standard practices and with guidance from the supervisor. In carrying out position duties, they will perform in accordance with applicable professional ethics and established Region Ten policies.
A bachelor’s Degree in the Human Services field is required. At least 1 year’s experience in the human services field or a combination of education and experience that is the equivalent is required; experience in developmental disabilities work and education is preferred. To ensure the safe and efficient operation of the program, a valid Virginia Driver’s License plus an acceptable driving record as issued by the Department of Motor Vehicles are required. For business use of a personal car, a certificate of valid personal automobile insurance must be provided.
