Case Manager I - Project Based Housing (ICMS)
St. Joseph Center is a nonprofit organization serving low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities. Since 1976, the Center provides comprehensive case management and integrated social services. The organization serves approximately 6,000 individuals annually and is affiliated with the Sisters of St. Joseph of Carondelet.

Position: Case Manager I — Project Based Housing (ICMS). Full-time, exempt.
Job Description
The Case Manager works as a team member with several social service professionals and paraprofessionals to ensure delivery of quality care in accordance with program rules and the Center’s mission.
Essential Duties and Responsibilities
Develop and maintain strong ties to the community, law enforcement, and other homeless service providers.
Link clients to bridge/interim housing programs and assist with completing required paperwork to enroll in permanent housing-related programs.
Provide case management and supportive services to meet the needs of each individual.
Build trust and rapport with individuals experiencing homelessness; assess clients using the Vulnerability Index (VI) SPDAT.
Link clients to physical health, mental health, and other supportive services; assist with enrollment in mainstream benefits and obtaining identification (e.g., California Driver’s License, Social Security Card).
Maintain organized case files and enter data into the Homeless Management Information System (HMIS).
Provide transportation assistance as needed (bus tokens/passes, taxi vouchers).
Participate in case conferencing and other community meetings; coordinate with outreach teams for joint efforts.
Qualifications
Knowledge, Skills & Abilities:
Highly motivated self-starter; ability to communicate with diverse groups and maintain strong organizational skills in a fast-paced environment.
Strong knowledge of homeless services and resources; experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
Bilingual ability is a plus.
Flexible hours including some early mornings and evenings.
Experience: Two years of experience providing services to highly vulnerable populations.
Education: Bachelor’s degree in Psychology, Social Work, Human Services, or a related field from an accredited institution.
Direct Reports: This position has no direct supervisory responsibilities.
Computer Skills: Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook); ability to use standard internet browsers.
Certificates, Licenses and Registrations: Valid California Class C Driver License or alternate transportation; valid automobile liability insurance.

Travel requirements as needed.
Work Environment and Physical Demands
