Overview
St. Joseph Center is hiring a Case Manager as part of the Full-Service Partnership (FSP) team. This role provides field-based, integrated case management to clients who experience chronic homelessness and severe mental health disorders. The Case Manager engages clients in the service continuum, supports housing readiness, and helps secure subsidized housing and permanent housing, while delivering ongoing intensive services to promote self-reliance and housing stability.

Responsibilities

Develop working relationships with other nonprofit agencies providing client services (e.g., Los Angeles Housing Authority and other partners).

Present findings to collaborative agencies to develop service plans that expedite housing opportunities and stability.

Collaborate consistently with the FSP team’s interdisciplinary members (MD, PMHNP, PA, MHT, and other case managers).

Assist with developing and implementing client treatment plans addressing mental health/substance use issues, relapse prevention, symptom/behavior reduction, medication management, and other risk factors affecting housing stability.

Conduct ongoing case management assessments to identify barriers to clients’ goals.

Identify and use client strengths to increase self-efficacy and independence.

Provide ongoing intensive case management that supports successful permanent housing, reduces social isolation, prevents relapse risks, and links clients to needed services (mental health, substance use treatment, life skills, budgeting, employment, etc.).

Provide crisis management as needed, including assessment of suicidal/homicidal ideation.

Communicate with landlords and collaborating agencies, responding to urgent requests in a timely manner.

Participate in meetings with collaborative agencies and community partners as directed.

Complete and maintain documentation per DMH and program requirements.

Qualifications

Knowledge, Skills & Abilities: Highly motivated self-starter; ability to communicate with a diverse range of clients and staff; strong organizational skills and the ability to work in a fast-paced environment.

Strong knowledge of homeless services and resources; experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.

A bilingual background is a plus.

Ability to work flexible hours including some early mornings and evenings.

Experience

Two years of experience providing services to highly vulnerable populations.

Education

Bachelor’s degree from an accredited college or university in Psychology, Social Work, Human Services or a related field.

Direct Reports

This position has no direct supervisory responsibilities.

Computer Skills

Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook).

Ability to use widely supported internet browsers.

Certificates, Licenses and Registrations

Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

Valid automobile liability insurance.

Travel Requirements

Must meet clients in the field.

Travel to meetings at various St. Joseph Center sites and at community partner locations as needed.

Work Environment
Work in a team setting and independently as required; reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. The typical work environment is moderate in noise.

Physical Demands
The physical demands include standing, walking, sitting, reaching, keyboarding, and occasional lifting up to 20 lbs.
