Job Scope: The Linkage Case Manager provides in-home and community-based skill building and linkage activities to adults with mental illness and co-occurring substance use disorders, primarily discharging from psychiatric hospital settings. Responsibilities include short-term case management, linkage to housing, medical and psychiatric services, and substance use supports. Services are flexible and delivered in various settings including hospitals, homes, and community spaces. The role involves coordination with KYC team members, hospitals, PAS agencies, and other community mental health services.

Clinical Duties:

Provide outreach, case management, linkage, and referral services following treatment plans for individuals discharging from hospitals, for approximately 180 days.

Complete in-person and phone sessions within 24 hours post-discharge, and ongoing as per treatment recommendations.

Meet clients at flexible times and locations based on their needs.

Coordinate discharge planning activities with hospitals and assessment teams.

Identify resources, provide referrals, and ensure clients are linked to needed services.

Support clients in locating and securing permanent housing, developing housing options, and collaborating with partners for resource access.

Perform crisis interventions as needed and support other community programs.

Consult with behavioral health staff to address clinical issues and coordinate care.

Provide immediate crisis support and coordinate with crisis teams and Living Room as needed.

Assist in training and orientation of new employees and participate in team meetings.

Administrative Responsibilities:

Participate in team meetings and generate referrals.

Complete clinical documentation and reports accurately and timely.

Communicate policies and procedures effectively.

Meet productivity targets and participate in training.

Assist clients with applications for assistance programs and benefits.

Education and Experience:

Master’s Degree in Social Work, Psychology, Counseling, or related field preferred (QMHP under Illinois Rule 132/140).

Preferred 1 year of experience working with adults with serious mental health issues.

Experience in non-traditional treatment environments preferred.

Valid IL Driver’s License and access to a vehicle.

Proficiency in basic computer and smartphone skills.

Schedule / Travel:

Full-time, 37.5 hours/week, with flexible schedule including occasional evenings and Saturdays.
