About The Job
Saint Joseph Health System is seeking a compassionate and detail-oriented RN Case Manager to coordinate patient care, utilization management, and discharge planning. This role ensures quality outcomes, efficient care delivery, and patient satisfaction through collaboration with physicians, departments, and families.
Key Responsibilities
Manage patient caseloads and coordinate care plans
Lead discharge planning and transition processes
Facilitate team meetings and resolve care discrepancies
Monitor length of stay and level of care using clinical criteria
Collaborate across departments to ensure continuity of care
Provide education and support to patients and families
Maintain compliance with healthcare regulations and documentation standards
Requirements
RN license in the State of Indiana
Bachelor’s degree in Nursing (BSN preferred)
3–5 years of clinical nursing experience; critical care experience preferred
Case management certification preferred
Strong communication, critical thinking, and computer skills
