Job Summary
The Case Manager for the Family Planning Program is responsible for providing comprehensive case management, education, and support services to individuals and families seeking reproductive health and family planning assistance. The Case Manager assists clients in identifying needs, accessing resources, and achieving self‑sufficiency while ensuring services align with Family Service Center’s mission and program goals. This role requires a compassionate, professional approach to client care, confidentiality, and advocacy.

Essential Duties and Responsibilities

Conduct intake assessments to determine client eligibility, health needs, and service goals related to family planning and reproductive health.

Develop individualized service plans focused on client education, health access, and overall wellbeing.

Provide information and referrals to community resources such as medical providers, counseling, and social support programs.

Follow up with clients to ensure continuity of care, monitor progress, and update case records.

Promote client understanding of family planning options, preventive care, and health education.

Conduct or assist with educational sessions on reproductive health, family planning methods, and available community resources.

Support outreach efforts to engage underserved populations and promote program awareness.

Collaborate with clinic staff and community partners to enhance service delivery and client engagement.

Documentation & Compliance

Maintain accurate, timely, and confidential documentation in accordance with agency, state, and federal guidelines.

Record all client interactions, referrals, and outcomes in the electronic database.

Ensure all required program forms, consents, and reporting data are completed accurately and on time.

Collaboration & Communication

Coordinate with Family Planning Program staff and other FSC departments to provide integrated client services.

Participate in program meetings and staff trainings.

Build and maintain relationships with local healthcare providers, social service agencies, and community organizations to expand referral networks.

Professional Conduct & Development

Adhere to Family Service Center’s policies, confidentiality standards (HIPAA), and ethical guidelines.

Maintain up‑to‑date knowledge of family planning practices, community resources, and program regulations.

Participate in ongoing training and professional development as required by the agency and funding sources.

Qualifications
Education: Bachelor’s degree in Social Work, Psychology, Human Services, Public Health, or a related field required.

Experience: Minimum of 1–2 years of experience in case management, family planning, health education, or social services.

Knowledge & Skills

Understanding of reproductive health, family planning services, and community health principles.

Strong communication, documentation, and organizational skills.

Ability to work with diverse populations in a culturally sensitive manner.

Knowledge of local community resources and service networks.

Proficiency in Microsoft Office and electronic case management systems.

Working Conditions

Work is performed in an office and clinic environment with occasional community outreach.

Regular interaction with clients, staff, and partner agencies.

May require occasional evening or weekend hours for outreach or special events.

Physical Requirements

Ability to sit, stand, and travel locally for client or community visits.

May occasionally lift or carry up to 20 lbs (educational materials, files, or supplies).
