Position Summary
The Case Manager plans, implements, and coordinates a comprehensive, individualized array of client services during residency and prepares clients for continuing care and safe, sober housing after discharge. This role also delivers psychoeducational and motivational enhancement instruction in group and individual settings, and performs administrative duties to support the program.

Responsibilities

Perform patient screening, intake, orientation, and assessment.

Develop or refine an individualized service plan based on assessment and other collateral data; make referrals, arrange transportation, and follow up on referrals during residency and for at least 30 days after discharge.

Provide individual case‑management sessions as required by the level‑of‑care and document the review in the client’s record.

Assist patients in developing motivation for recovery and engaging in recovery activities.

Assist patients in identifying and linking to appropriate resources for immediate needs and for subsequent levels of continuing care, safe and sober housing, and other recovery supports.

Conduct psychoeducational and motivation‑enhancement group sessions.

Attend staff meetings, case conferences, and required training to coordinate with the program team and ensure quality of care and continuity in accordance with clinical policy.

Maintain working relationships with agencies, community resources, and families to coordinate services; provide information on substance‑abuse treatment resources; make referrals; facilitate follow‑up to ensure appropriate services delivered.

Other duties as assigned.

Qualifications

Education: Bachelor’s Degree from an accredited institution or an equivalent combination of education and experience.

Experience: Knowledge of and experience with case management; experience with substance‑abuse, mental‑health, health, housing and community support services.

Strong interpersonal and analytical skills; detail oriented; able to work under pressure, meet deadlines, adapt to changes, and demonstrate dependability.

Exceptional customer/client service skills with the ability to resolve service issues.

In‑depth knowledge of Joint Commission and DHS standards.

Ability to use logic and reasoning to identify strengths and weaknesses of alternative solutions and handle multiple priorities with a sense of urgency.

Work Environment
This position is an in‑person office role. Daily face‑to‑face interaction occurs with staff and clients. The workday often includes sitting at a desk, working on a computer, meeting with clients, and completing paperwork.
