Job Description
MH: Case Management and Social--(Unscheduled, Days) -
Job Summary: The Case Manager serves as a key member of the patient care delivery team, working efficiently in a fast-paced environment. They utilize clinical expertise, discretion, and independent judgment in performing the nursing process (assessment, care planning, intervention, evaluation) within a patient-centered care professional practice model (Duffy's Quality-Caring Model). They maintain knowledge of specialized case and utilization management methodologies, ensuring medical appropriateness criteria are met from admission through discharge. The role involves overseeing interdisciplinary care plans, discharge planning, and reevaluation based on patient needs. The Case Manager advocates for nursing as an empowered profession, embracing new knowledge and serving as a change agent. They participate in committees, task forces, and staff meetings, and perform other duties as needed.
Education and Experience: Bachelor of Science in Nursing (BSN) required. Minimum two years of hospital nursing experience required. Preferably, two years of case management, utilization review, or discharge planning experience in an acute care setting.
Knowledge and Skills: Basic knowledge of case and utilization management practices, healthcare regulations, and accreditation. Proficiency in English; Spanish preferred. Competent in computer use, including Microsoft Office, electronic medical records, and reporting software. Effective communication, professionalism, critical thinking, independence, organization, problem-solving, attention to detail, and auditory/visual acuity are essential.
Licensures and Certifications: Active, unrestricted California RN license required. BLS certification from the American Heart Association required. Certification in Utilization Management or Case Management is preferred.
