Care Coordinator- Homeless Assessment and Referral (HAR)
The Rescue Mission of Trenton is seeking a Care Coordinator - Homeless Assessment and Referral for our Emergency Shelter Unit. Under the direction of the Coordinator of Case Managers and Open Access, the HAR Care Coordinator (Case Manager) will provide direct case management and supportive services to emergency shelter clients.

Rescue Mission of Trenton was founded in 1915 and is a non‑profit organization that helps rescue people from themselves and their situations. Our dedicated staff in the Shelter and our Behavioral Health Unit offer care, support and encouragement to all that reside here.

Qualifications For Position
A Bachelor’s degree preferred; or High School Diploma or Certified Peer Recovery Specialist Certification
Candidate must have a minimum of three (3) years’ experience in case management. Preferably prior experience with homeless individuals.
Should be an active learner with great listening skills, critical thinking, monitoring, reading and written comprehension, and understanding of addictions.
Must be able to operate a computer, be proficient in Microsoft Office (Word and Excel), Homeless Information Management System (HMIS) and must have a valid driver’s license.
Provide direct case management services and support to individuals and clients, such as managing referrals and resolving issues.
Collaborate with key partners, including financial, housing, substance abuse treatment plans, and criminal justice agencies, in accordance with Rescue Mission policies, procedures, guidelines and applicable state regulations.
Direct case management and supportive services to assigned clients and be able to participate, and encourage client participation, in a team approach to treatment including coordinating client services with other Rescue Mission staff and other provider agencies.
Assess individuals with a criminal history and their need for substance abuse treatment services, develop, implement, review, revise treatment plans under supervision and monitor client outcomes.
Document and maintain client charts, roster, records, and reports.
Understand, research and analyze community needs to provide program direction and goal recommendations while being an advocate on client behalf, policy, and program level.
Participate in staff meetings, determine the organizational policies regarding eligibility, program requirements, and program benefits, communicate program trends to supervisor, participate in ongoing training and education to maintain or obtain certification.
