Case Manager – Hep C Program
Brandywine Counseling & Community Services, Inc. (BCCS) is seeking a dedicated Case Manager to lead our Hepatitis C (HCV) program. BCCS has served the Brandywine community since 1985, providing comprehensive substance abuse and behavioral health treatment. Our mission is to empower individuals and families affected by mental illness, substance use, HIV, and other complex challenges.
Responsibilities
Facilitate referrals and coordinate care for clients with SUD, HCV, Hepatitis B, SMI, and related social services to address social determinants of health.
Design, develop, and supervise case‑management policies, after‑care plans, and follow‑up procedures for the program.
Provide specialized case management and aftercare services, conducting regular follow‑ups and counseling sessions as needed.
Supervise community visits, manage a caseload of active clients, and conduct group and individual counseling to help clients address complex issues.
Monitor clients’ emotional and psychological status, arranging referrals to ancillary services when appropriate.
Conduct workshops and seminars for clients and community members, maintaining relationships with referral sources and partner organizations in health, addiction, and vocational services.
Write detailed reports, maintain accurate records, and assist other staff with discharge planning and aftercare coordination.
Develop and implement procedures to complete clinical records and correspondence while ensuring compliance with confidentiality procedures and applicable state and federal regulations.
Occasional driving may be required to conduct therapeutic community sessions.
Knowledge, Skills, and Abilities
Establish and maintain effective working relationships with community agencies, organizations, and the public.
Demonstrate knowledge of Hepatitis C and intervention/treatment strategies.
Speak and write effectively with clients and stakeholders.
Requirements
Associate’s degree required.
Minimum of two years’ experience in addiction or a related field required.
Valid driver’s license with two points or less required.
Schedule
Monday – Friday
7:00 a.m. – 3:00 p.m.
