Responsibilities
Manage resources, coordinate patient care from admission to post-discharge, and oversee interdisciplinary plan-of-care decisions.
Work with an interdisciplinary team to plan and execute patient discharge experiences and care plans.
Coordinate with the team to establish tentative discharge plans and contingency plans; participate in planning and execution of the patient discharge experience.
Monitor patient experience, quality, timeliness, service appropriateness, and payor requirements; participate in utilization review and case management reporting.
Facilitate weekly team conferences and coordinate all treatment plan modifications; complete case management addendums and required documentation.
Maintain knowledge of regulations, company policies, procedures, and department operations; ensure accuracy of discharge and payor-related information in patient records.
Attend Acute Care Transfer (ACT) meetings, identify trends, and collaborate to reduce ACTs.
Meet with patient/family within 24 hours of admission and assess goals; complete case management addendum within 48 hours of admission.
Educate patient/family on rehabilitation and the Case Manager role; establish a communication plan; schedule and facilitate family conferences as needed.
Assist patients in procuring resources to avoid discharge delays; monitor compliance with regulations for orthotics and prosthetics ordering and payment.
Make appropriate and timely referrals and document post-discharge providers/physicians; participate in the case management on-call schedule as needed.
Participate in the utilization review process and review case management data to plan actions.
Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of the discipline (e.g., RN, SW, OT, PT, SLP, Rehabilitation Counseling).
If licensure is required for the discipline within the state, hold an active license.
Must meet eligibility requirements for CCM or ACM certification upon entry or within two years of entry; CCM/ACM certification required or must be obtained within two years of being placed in the Case Manager II position.
Minimum Qualifications:
For Nursing: Associate Degree in Nursing with RN licensure; BSN preferred. A diploma is acceptable only in states where it is the minimum licensure requirement.
For all other eligible licensed health professionals: minimum of a bachelor's degree; graduate degree preferred.
2 years of rehabilitation experience preferred.
