Overview
BCCS seeks a Case Manager for our Hep C Program. BCCS is a trusted provider of substance abuse and behavioral health treatment, serving families and individuals with mental illness, substance abuse, HIV, and co-occurring diagnoses.
Responsibilities
Facilitate referral and system navigation for SUD, HCV, HIM, SMI, and social services, addressing social determinants of health.
Design, develop, and supervise case management, aftercare, and follow‑up policies and procedures for the program.
Provide specialized case management and aftercare services, conducting scheduled follow‑ups.
Supervise active clients, conduct group and individual counseling, assist clients in achieving adjustments to complex problems.
Monitor clients’ emotional/psychological status and arrange referrals to ancillary services.
Conduct workshops and seminars, maintain relations with referral sources and community organizations for in‑treatment and aftercare planning.
Write reports, maintain necessary records, and assist staff in discharge and aftercare planning.
Develop procedures to complete clinical records and correspondence while maintaining compliance with confidentiality procedures and state/federal regulations.
Occasional driving to conduct therapeutic community sessions.
Qualifications
Associate Degree required.
Minimum 2 years’ experience in addiction or related field required.
Valid driver’s license with 2 points or less required.
Knowledge of Hepatitis C and intervention/treatment strategies.
Ability to establish and maintain effective working relationships with community agencies, organizations, and the public.
Strong written and verbal communication skills.
Schedule
Monday – Friday, 7:00 a.m. – 3:00 p.m.
