Care Manager Rn

RN Care Managers in the acute care setting provide a variety of services to appropriately help progress patients through the continuum of care. Key services include screening for post-acute needs, transition planning, care coordination, education, community care referrals, care planning in accordance with regulatory requirements, collaboration with the continuum care team, ongoing monitoring of plan effectiveness, and reassessment of the need for Care Management (CM) services.

Positions Available:

  • Full Time with 1 FTE with Day Shift Schedule
  • Part Time with 0.6 FTE with Day Shift Schedule

Required qualifications:

  • Nursing degree/diploma required upon hire
  • Bachelor's degree in Nursing or higher within 3 years of hire
  • Oregon Registered Nurse License upon hire
  • 2 years Acute care hospital or community based experience
  • 1 year Experience with electronic medical records or documentation systems
  • 1 year Home health, mental health, substance use, hospice, utilization review, discharge planning, or care management experience

Preferred qualifications:

  • Bachelor's degree in nursing upon hire
  • CCM certification upon hire

Pay Range: $55.67 - $81.25
