Social Worker (Program Coordinator) – Community Residential Care Coordinator
Responsibilities
Provide advocacy, linkage, and referral services on behalf of chronically mentally ill.
Provide intake, screening, referral and case management services.
Assist in maintaining an active liaison with community agencies serving the “high risk” population with modified PACT program.
Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS rules and regulations.
Attend and regularly present cases in the treatment team meetings.
Read and be knowledgeable of all current CREOKS policies and procedures.
Maintain productivity averages of direct service to clientele as set by CREOKS for full‑time employment.
Coordinate and manage indigent medication/patient assistance program.
Turn in billing and associated documentation as mandated by CREOKS policy.
Understand and follow all administrative, clinical and programmatic policies and procedures of the agency.
Provide Psychiatric Social Rehabilitation Services as assigned by Supervisor to agency clients.
Attend and present cases in the treatment team staffing conferences.
Maintain full‑time caseload as assigned by the Site Director and meet the daily productivity average set by Administration.
Complete daily billing documentation and clinical documentation preferably before the next work day.
Interact with agency client families and other agencies to ensure appropriate therapeutic treatment.
Participate in mandatory trainings such as CAPE and CPR.
Implement rehabilitative curriculum and develop support materials based on best practices and evidence‑based treatment models.
Teach rehabilitative skills focusing on activities of daily living and successful integration into community life, including independent living, self‑care, social skills, lifestyle changes, and recovery practices.
Treat all types of clients within the community based on level of staff competency.
Complete all other duties as assigned.
Qualifications
Minimum of a Bachelor’s degree in a mental health related field or a Bachelor’s degree plus at least 2 years supervised experience in a treatment setting.
Maintain current case management certification.
Basic understanding of social and cultural factors influencing human behavior.
Knowledge of various treatment modalities.
Capability to work as part of a team.
Core competencies related to individuals with co‑occurring disorders.
Case Management certification (if applicable).
