The Case Manager , in accordance with the Joint Commission, federal, and state regulations, Freedom's mission, policies and procedures, and PI standards, is responsible for coordinating with the admission staff and clinical staff to facilitate meeting patients' treatment needs. The Case Manager assumes responsibility for managing the discharge plan and utilization review activities throughout the patient's stay in the program. The Case Manager contacts referral sources and family members to gather clinical information for the multidisciplinary team and reports to the treatment team. They interact with members of the medical/clinical team to ensure effective communication. The Case Manager accurately documents findings and data in the medical record that support the level and intensity of services rendered. They function as a member of the multidisciplinary team and assist in facilitating the treatment team process. The Case Manager is the primary contact for payor sources and liaises with medical and clinical staff to communicate admission and continued stay criteria to referral sources, families, and payors as needed. They communicate with patients, families, and referral sources to ensure positive treatment outcomes. Additionally, they maintain performance improvement activities within the department and participate in QM activities. The Case Manager adheres to the highest ethical standards regarding patient care and rights. They facilitate communication with outside agencies to ensure patient rights, care, and needs are met.
Education: Some college; a psychology major is preferred or nursing education.
License: None required.
Experience: 1+ years coordinating utilization review and discharge planning activities.
