Overview
Case Manager / D&A Specialist role at Mental Health Partnerships . The Targeted Case Manager – D&A Specialist provides services to participants living in the community with persistent mental health and drug and alcohol challenges. After assessing needs, the D&A Specialist helps the participant obtain resources and services in the community, in a recovery‑focused partnership that helps participants achieve their goals for community living. The D&A Specialist also assists homeless individuals to locate shelter/residence and engages individuals who are incarcerated, acting as a community link in reestablishing a productive life within the community after release.

Responsibilities

Provides assessment of substance use to determine the need for treatment and other services including specialized housing and other recovery supports in the community.

Develops individualized plans with the participant and other members of the case management team to address substance use treatment needs.

Links the participant with appropriate substance and alcohol use services and monitors these services with other case management team members.

Provides consultation and support to case managers in identifying and engaging individuals who are experiencing substance use issues.

Supports individuals in the community with obtaining necessary substance and alcohol use treatment.

Facilitates a weekly Drug and Alcohol Recovery group.

Continually engages ambivalent individuals to assess readiness for treatment or services, assesses their stage of change, and works with them using motivational interviewing.

Conducts outreach to locate individuals on their caseload who may be missing and have concerns about substance use.

Collaborates with participants to identify, develop, and access resources and supports to increase self-sufficiency, community integration, and inclusion.

Collaborates with participants to assess hopes, strengths, accomplishments, and challenges, and creates goals; assists in identifying supports and arranging services to achieve those goals.

Supports, teaches, and models recovery, recovery tools, personal responsibility, self-advocacy, and hopefulness.

Coordinates and/or selects relevant supports to help participants develop self-directed recovery and crisis plans; facilitates self-review of progress at each encounter.

Provides crisis intervention and de-escalation as needed.

Obtains necessary information during the intake process and documents participant progress in MHP’s electronic health records system; verifies information with the participant for accuracy.

Supports and assists participants in the community and ensures safety during events, visits, and other interactions.

Complies with established quality and regulatory standards, including supervision and training requirements.

Applies general knowledge of recovery services to complete small projects or tasks with limited supervision.

Creates weekly schedules, two weeks in advance, that meet program productivity standards and supervision.

Maintains compliance with all MHP policies and procedures and reports policy violations to management or Human Resources as required.

Other duties as assigned to fulfill MHP’s mission.

Requirements

Bachelor’s degree with major coursework in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling or education; or be a registered nurse whose duties are limited to TCM responsibilities only.

Minimum of 2 years of experience in a behavioral health work environment with a focus in substance use.

Knowledge of local community and statewide substance use treatment facilities and resources.

Knowledge of treatment techniques and procedures as applied to a substance or alcohol use.

Must have a BHTEN certification or be able to obtain one within one year of hire date.

Must possess a valid driver’s license.

Must be able to qualify for and obtain a prison ID.

Must possess basic computer skills to perform job duties, including desktop computing, e‑mail, timesheet management, electronic health records, internet research, etc., and the ability to learn software applications relevant to the position.

Must have access to a car to use for work on a daily basis.

Must have automobile insurance coverage at state requirement levels and current state registration.

Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.

Must be able to work under pressure and meet deadlines and billable unit requirements while maintaining a positive attitude and providing exemplary customer service.

Strong skill sets in oral and written communication, cultural competence, interpersonal relationships, and teamwork, attention to detail, customer service, time management, autonomy, problem-solving, and organization.

Salary
$24.00 per hour

Location & Schedule
The D&A Case Manager is a full‑time position, Monday - Friday, 8:30 AM - 5:00 PM (with the opportunity to create a flexible schedule with manager's approval) and is located at 414 East High Street, Pottstown, PA 19464 . This is a hybrid work-from-home position, with required in-person participant visits and office days and the ability to work from home one day per week.
