About the Role
As part of PATH’s Interim Housing Team, the Case Manager assists clients in breaking the cycle of homelessness through a “whatever it takes” approach. The main focus is to develop Individualized Service Plans and connect clients to resources that achieve housing goals and improve overall well‑being.
Position Responsibilities
Conduct intake and individualized needs assessment for all clients and develop Individualized Service Plans (ISP) addressing housing, income, and personal goals.
Provide referrals, linkages, and support to resources that help clients achieve ISP goals.
Complete program specific assessments (e.g., VI-SPDAT).
Evaluate strengths and challenges, conduct 90‑day reviews with clients after initial intake.
Maintain accurate, timely documentation per PATH policies and HIPAA requirements.
Maintain confidentiality of client files.

Preferred Qualifications
Minimum of 2 years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
Minimum Qualifications
All levels of education and experience are welcome.
Mandatory Requirements
Employment eligibility verification.
CPR/First Aid training or willingness to obtain.
Completion of Tuberculosis Test, Background Screening, and Drug Test.
Driving
Driving is an essential function. Candidates must have reliable transportation, a valid driver’s license, proof of insurance and vehicle ownership, and ability to qualify for PATH’s insurance coverage.
