Case Manager (CM) Inpatient Rehabilitation
Facility: Mercy Rehabilitation Hospital South (IRF)
Position: Case Manager (CM)
Shift: Full Time Days
Schedule: Monday - Friday, 8:00 am – 5:00 pm with flexibility
Licensure: Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
Responsibilities
Demonstrate use of effective case management skills to plan, coordinate and evaluate patient care and movement in the care continuum
Assist with the development of individualized care plans
Establishes and maintains a therapeutic relationship with patients and families
Collect quality review data
Participate in survey preparation activities
Interact with assigned program consultant
Participate in unit safety committee activities
Contribute to the delivery of cost-effective care
Participate in continuing education/professional development activities
Communicate and demonstrate a professional image/attitude for patients, families, coworkers and others
Adhere to policies and practices of applicable professional organizations, client hospital and Lifepoint
Support the team in the development of student programs
Maintain competency, licenses/certifications, and memberships in applicable professional organizations
Other duties as assigned
Qualifications
Healthcare professional with clinical experience including med-surg and post-acute care settings
Current license/certification to practice within their skill set
Registered nurse strongly preferred; other licensed professionals or certified case managers acceptable
Demonstrated progressive responsibility, positive communication skills and basic computer word processing ability
Knowledge of reimbursement benefits and requirements including Medicare, Medicaid and managed care

Mercy Rehabilitation Hospital South is a 50-bed Rehabilitation Hospital providing inpatient rehabilitation services for patients recovering from strokes, brain or spinal cord injuries, amputations, complex orthopedic injuries and other conditions.
