Case Manager – Children’s Residential Program
Salary: $47,493.33 – $58,000 per year (depending on experience)

Purpose : To initiate, facilitate, and perform a variety of casework related functions pertaining to children residing in the children’s residential program.

Key Responsibilities

Carry a caseload of children living in assigned homes, residing in the residential homes on Palatine Campus and in the community.

Conduct at least weekly visits with each assigned child in their residential home to ensure appropriate services are implemented and to collaborate with all staff related to the child’s overall care, progress, and treatment.

Secure documentation, compile historical information, assess case situations, and make recommendations regarding children on caseload.

Collaborate with the interdisciplinary team in developing and implementing the Mental Health Assessment and Individual Treatment Plans for all cases, including quarterly and six‑month reports.

Attend IEPs and advocate for children in the school system.

Attend monthly team meetings for the homes assigned.

Monitor and supervise the placements and adjustments of children. Responsible for permanency planning and discharge planning for children.

Attempt and maintain regular, ongoing contacts with the parents/guardians of children. For DCFS wards, make all efforts to establish a relationship with the biological family and assist the biological family in reaching reunification where applicable.

Responsible for the upkeep of children’s files, which includes obtaining consents, monitoring and completing progress notes, and securing all required documentation for assessment and provision of services to the child.

When applicable, work in conjunction with the DCFS worker for implementation and coordination of sibling and birth parents’ visits.

Ensure that licensing standards and applicable laws, rules, and policies are followed.

Assist with the intake process of new children moving into assigned homes, including setting up the child’s file and following up to ensure all necessary documentation is received.

Work cooperatively with other staff in Children’s Services, DCFS, DHS, and collateral resources.

Facilitate 6‑month staffings, attend staff meetings, committee meetings, supervision, and trainings as required.

Ensure safety activities in the workplace and immediately report all accidents and safety hazards to your supervisor.

Successfully complete Little City Foundation required training, re‑training, and any additional training as required by policies and practices, or as assigned by your immediate supervisor.

Perform other duties as assigned by your supervisor.

Minimum Qualifications
Must have a college or university degree that meets the federal educational standards required to serve as a Qualified Intellectual Disability Professional (QIDP) and have at least one‑year experience working directly with individuals with developmental disabilities. Must be at least 21 years of age. Must have excellent verbal and written communication skills as well as organizational skills. Must be capable of transporting children and possess a current IL valid driver’s license.

Physical Requirements & Work Environment
Responsibilities include sitting, walking, standing for prolonged periods; frequently grasps, lifts, holds, or feels objects; occasionally stoops, kneels, crouches, or crawls. Must use manual and finger dexterity and eye‑hand coordination. Able to assist and lift up to 50 pounds with varying assistance. Must have corrected vision and hearing within normal range and ability to operate a motor vehicle. Must be able to perform CPR and CPI. Exposure to blood, body fluids, infectious substances (Category III). Exposure to cleaning agents and disinfectants. Residential work environment with noise level from normal to loud.
